How to File an Exemption in the State of Texas
- Step One: Visit DSHS Texas — School Exemptions
- Step Two: Submit your request via the DSHS Conscientious Objection Request portal
For Step 2, order 5 forms per year every January and only have them notarized when an establishment asks for them. Notarization is normally free at your bank. You will also need a statement of health from your provider; turn both into the school. Some pediatric offices deny care to patients who choose not to vaccinate — they no longer receive insurance kickbacks for those patients. You can use CMS Open Payments to look up specific providers.
